Patient Registration
The Practice will register and provide medical services to patients living within the following boundaries :
- West : River Severn
- South : A4440 - Broomhall Way / Crookbarrow Way
- East : M5 Motorway
- North : Pershore Lane to Martin Hussingtree then across to Hawford.
(Including Fernhill Heath).
People wishing to register with the Practice should speak to one of our receptionists who will provide the necessary forms for completion.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
