Patient Responsibilities

The Doctors and Staff at Elbury Moor are continually striving to provide high quality services for its patients. In order for us to achieve this we would ask you to observe the following:

• Please keep your appointment and attend on time
• If you are unable to keep your appointment please tell us
• Prepare for your appointment
• Do not ask for a home visit unless you are too ill to attend Surgery
• Please allow 48 hours for repeat prescriptions
• Keep us and the local hospital informed of changes in your name, address + phone number
• Ring for home visits before 10.30am
• If the Doctor is running late please be patient
• Please remember that an appointment is for one person only – please book a separate consultation for each member of the family

Disclosure of Patient Information

The Practice is registered under the Data Protection Act and also under the Freedom of Information Act 2000 for disclosure of patient information.

The Doctors and Staff at Elbury Moor Medical Centre will try to give you the best possible service at all times.  However, there may be occasions when you feel it necessary to raise a concern.  This leaflet will explain what to do if you have a complaint about the service we provided for you.

We welcome any comments which you feel would benefit the practice and improve patient care.  Complaints, sugggestions and constructive criticism, including compliments can be valuable in developing a better quality of service to our patients.

Elbury Moor Medical Centre is an approved training practice and currently provides a training facility for registrars and final year medical students.

Registrars are fully qualified doctors who spend a year working in general practice as part of their training to become a General Practitioner.

Final Year Medical students are currently studying at Birmingham University and spend 11 days at the practice learning and observing the role of the GP.
